Change general practitioners

You may change general practitioners twice a year on  April 1^st and October 1^st. This only applies to Bonaire. Your change must be registered in ZJCN's system before these dates. To do so, you must complete the mutation/registration form and return it to ZJCN on time.  Click here to download the form. Don't forget to inform both your previous GP and your new GP.
