Residents of Bonaire who have been insured with ZJCN for at least two years are invited to collect their Health Card.
From the 28th of October, Care and Youth Caribbean Netherlands (ZJCN) will introduce the healthcare card on Bonaire as replacement for the paper insurance certificate. Previously, you needed to collect an insurance certificate from the ZJCN office before travelling abroad. With the introduction of the healthcare card, this is no longer necessary. Bonaire residents with health insurance can collect their card from specified locations.
It is necessary to fill in the customer form beforehand and bring it along when collecting your healthcare card. Don’t forget to bring your ID card (sedula).
The healthcare card is personal and serves as proof that you are registered with ZJCN. With the healthcare card, it is no longer necessary to request an insurance certificate from the ZJCN office before travelling abroad. The card contains ZJCN’s contact details, so you can easily communicate with our office. Additionally, the healthcare card can also be used locally at GP practices, hospitals, pharmacies, and labs.
While the healthcare card proves that you are registered with ZJCN (an insurance certificate), it is not a travel insurance or a guarantee letter. If, during your stay abroad, you face an unexpected medical emergency, such as a serious illness or hospital admission, you should contact ZJCN as soon as possible. Your healthcare card will display your first name, last name, ID number, date of birth, and the island where you reside. Each legal resident of the Caribbean Netherlands who have been insured with ZJCN for at least two years will receive a personal healthcare card. Parents of children under 18 years old can collect their child’s card.
Authorisation Form
Residents with a disability can authorise a family member to collect the card on their behalf via an authorisation form, available both online and at the ZJCN office. When submitting the authorisation form, you must bring both the original ID card (sedula) of the client and the family member. Also, remember to bring the completed customer form.
To collect the card, you must have a valid ID card (sedula) and the completed customer form. We also request that you provide your bank account number so that we can register it in our system. This is not mandatory, but facilitates the reimbursement of any future claims. Additionally, we need your current phone number and email address to keep your details up-to-date in our system. There is no charge for obtaining the healthcare card, though fees will apply for card replacements.
