White and yellow card
The 'doctor's card for claim for lost wages' is generally known as the white or yellow card. The yellow card is used for sickness and pregnancy. The white card is used for industrial accidents. The employer has these cards available in case they are required.
The white card is the accident card. If an employee has been involved in an industrial accident, the employer gives him or her a white card together with the report form for industrial accidents. The employer fills in the details of the employee and the company in the white card. It is important to make sure that this card is filled in completely or the employer will not be able to claim lost wages. The employer can obtain blank white cards at the SZW unit. The report form is filled in by the employer and the first treating physician (GP or first aid doctor).
The yellow card is the sickness card. If the employee calls in sick, the employer gives him or her the yellow card on the first day of sickness. If the employee is pregnant she is given the yellow card before she takes maternity leave. The employer fills in the details of the employee and company in the yellow card. It is important to make sure that this card is filled in completely or the employer will not be able to claim lost wages. The employer can obtain blank yellow cards at the SZW unit.
Transition to online application
The yellow and white cards will disappear in the course of 2021. The SZW unit will soon switch to a digital web portal. The employer can then report employee absenteeism via this web portal. The RCN unit SZW treats the sick leave notification via the web portal as an application for loss of wages. Please keep an eye on our website for more information.
