Collect Your Healthcare Card During Walk-in Hours at the ZJCN Office
Kralendijk – Haven’t picked up your healthcare card yet? Care and Youth Caribbean Netherlands (ZJCN) invites you to do so. The healthcare card is intended for residents of the Caribbean Netherlands who have been insured with ZJCN for at least two years. It replaces the paper insurance certificate and simplifies the healthcare process, especially in case of urgent medical assistance abroad.

To make collecting your healthcare card easier, you can now visit our office at Kaya Grandi 91 during our fixed walk-in hours:
- Monday, Wednesday, and Friday: 08:00 – 12:00
- Tuesday and Thursday: 13:00 – 16:00

It is also possible to make an appointment in advance. Send an email to zorgpas@zorgverzekeringskantoor.nl with your full name, sédula number, and phone number. We will contact you as soon as possible to arrange an appointment.
What Do You Need to Bring?
To collect your healthcare card, you will need a few documents. Bring your sédula and a completed customer form. You can find this form at www.zvkcn.nl or at our office. Parents or guardians can collect the health insurance card for minor children; a passport is also allowed for minors.
If you are unable to collect the healthcare card in person, someone else can do so on your behalf. The authorized person will need to bring:
- A signed authorization form, completed by both you and the authorized person. This form is available at www.zvkcn.nl or at our office.
- Your original sédula.
- The ID of the authorized person.
Important to Know
The healthcare card is not a replacement for travel insurance. Therefore, don’t forget to take out additional travel insurance before traveling abroad to ensure full coverage in case of medical emergencies.
